The following information is to assist you in preparing for the funeral arrangement conference:
BURIAL CLOTHING
Burial clothing should consist of all underclothing as well as the desired outer burial clothing. Shoes are not required.
PERSONAL ACCESSORIES
These items include glasses, all forms of jewelry and any other items to be placed on the deceased or in the casket. Please inform the funeral director at the time of the conference if you desire any of these items to be removed prior to burial.
BIOGRAPHICAL INFORMATION
Biographical information will be taken at the conference to assist the funeral home in the preparation of a newspaper obituary and in the completion of legal documents such as certified death certificates.
Information Needed:
MILITARY DISCHARGE PAPERS
A copy of the discharge papers (DD-214 form) will be needed at the time of the conference if a military flag and flag presentation are desired as a part of the funeral ceremony. The papers will also be necessary if a government grave marker is desired.
SOCIAL SECURITY
The social security number of the deceased will be needed at the time of the conference. Notification of the death to the Social Security Administration will be completed by Cox Memorial Funeral Home & Crematory.
PHOTOGRAPHS
Photographs of the deceased and his/her life will be produced into a loving video tribute as a gift from the funeral home to the family. The number of photos for the tribute range from 15 to 24 photos. The photos can be of any quality with the size being 8 X 10 or smaller. Photos will also be used in the production of the personalized memorial folders that are handed out at the funeral service and may be used as part of the newspaper obituary.
MUSIC
Musical selections will be discussed as part of the funeral ceremony. If you have any certain compact discs/tapes/music that you would like for the ceremony, bring them with you if they are available. Selected musicians may also be desired to fulfill the musical portion of the service.
MINISTER/OFFICIANT
A minister or funeral officiant will be needed to assist in the funeral ceremony. If the family has no preference of a minister, the funeral home will be of assistance in obtaining a minister or officiant.
PALLBEARERS
Active pallbearers may be desired to assist in the transporting of the casket and honorary pallbearers may be desired as an act and honor of friendship.
MEMORIAL
A memorial in honor of the deceased may be selected for those desiring to give a contribution to a certain charity, cause or organization.
CERTIFIED DEATH CERTIFICATES
Certified death certificates may be required to meet legal obligations in the settling of personal estates, trusts or wills. The most common use of certified death certificates includes life insurance policies, transfer or sale of real property, etc.
FLOWERS
Flowers may be arranged through Cox Memorial Funeral Home & Crematory or can be obtained through the floral shop of your choice.
MONUMENTS
A permanent monument or a temporary grave marker may be obtained through Cox Memorial Funeral Home & Crematory. Should a monument already be in place at the cemetery, the funeral home can arrange to have the death date engraved.
LIFE INSURANCE POLICIES
Should a life insurance policy be the desired method of caring for the funeral expenses, the policy will be needed in the filing of all claims regarding the policy. The funeral home will assist in the filing of all documents.
